Have you ever encountered the original index to The Decline and Fall of the Roman Empire? IIRC, it was created for the first complete edition -- very 18th century, from when indexes were new creatures. Nothing as terse as modern indexes. Penguin Classics reproduces it, with page numbers of that edition. 'Tis wonderful stuff.
